Health and injury information consent

The Built Mthd is an adult online coaching service, not emergency care and not a substitute for diagnosis or treatment by a qualified health professional. In an emergency in Australia, stop using the service and call 000.

What you are being asked to consent to

Your Coach may ask for limited health, injury, symptom, physical-limitation or body-measurement information in check-ins, progress records or secure messages only where it is reasonably necessary to deliver and safely adapt your personal-training service. Do not send full medical records or information that is not needed for coaching.

Who can use it

Liam Robbert David Smit, trading as The Built Mthd, authorised Admins and your assigned Coach may use this information for your coaching and service administration. Approved service providers may process the minimum information needed to operate and secure the portal as described in the privacy notice.

Your decision

Consent is voluntary and is recorded only after you select each unchecked confirmation in the client portal. If you decline or later withdraw, personalised programs, habits, check-ins, progress collection and new coaching activity pause. You keep access to support and may request access/export, correction or account closure.

Withdrawal

You may withdraw future consent from the portal account menu or by emailing coach@thebltmthd.com. Withdrawal stops new sensitive-information collection and personalised coaching. Existing records follow the retention schedule unless deletion is required or continued retention is legally permitted or required.
